The Indiana State Police have launched an investigation into an officer-involved shooting that led to the death Tuesday night of a 23-year-old North Vernon woman.

Rachel Blake, who reportedly denied reported orders to drop a knife she was holding, was shot by two North Vernon police officers as she advanced toward them from inside a bathroom at a home on Thomas Street, according to a news release from Sgt. Stephen Wheeles.

The public information officer for the Versailles Post reported both officers reportedly told Blake to drop the knife and also used Tasers in an attempt to disarm her. They eventually fired at Blake who was struck at least once during the incident reported shortly after 7 p.m.

Officers immediately began administering first aid to Blake and Rescue 20 emergency medical service personnel also were called to provide advanced medical treatment, Wheeles said.

Blake was pronounced dead at the scene by the Jennings County Coroner’s Office.

The state police agreed to conduct the investigation after being requested to do so by North Vernon Police Department. Officers with the Jennings County Sheriff’s Department also initially responded to the initial incident.

Both North Vernon officers, who were not named, have been placed on administrative leave per department policy.

The investigation is ongoing.  At the conclusion of the investigation, the case will be presented to the Jennings County Prosecutor’s Office for review.
