An Indiana State Department of Health and Optum COVID-19 testing site is located on the third floor of the Schneck Professional Building, 411 W. Tipton St., Seymour.

Testing is available from 8 a.m. to 8 p.m. Monday through Friday by appointment only. Results typically come back from the lab within 72 hours.

There is no out-of-pocket expense for the test. Those with and without health insurance can be tested.

Those wishing to be tested need to park on the third floor of the parking garage and access the third floor through the sliding glass doors. There is no access from within the hospital.
