Rita Kay Handley-Chapman, 64, of Seymour died Sunday, May 29, 2016, at Kindred Transitional Care and Rehabilitation of Columbus.
Survivors include son, Jim (Heather Bell) Handley of Seymour; three grandchildren; and a brother, Ronnie Pemberton of Florida.
A private graveside service will be conducted at Riverview Cemetery at a future date.
Memorials to the Rita Chapman Memorial Fund may be sent to Woodlawn Family Funeral Centre, which handled the arrangements.
